The Director, Care Management leads teams of nurses and behavior health professionals responsible for care management. The Director, Care Management requires an in-depth understanding of how organization capabilities interrelate across the function or segment.

The Director for the Edge Care team will serve as the leader for a multidisciplinary team providing whole person care for seniors with multiple chronic conditions and unmet social needs. This is a unique opportunity to help lead a team transforming the care that we provide for seniors, working to meet their social, emotional and medical needs, enabling them to live the life that they hope for. This role will serve as the lead architect of designing, executing and overseeing a new multidisciplinary care team in multiple geographies.
This role presents the unique opportunity to be a part of building an innovative clinical strategy and shaping the necessary tech-enabled services to deliver on that experience from the ground up. The role requires experience overseeing teams in both direct patient care and in call center environments and an understanding of how we can provide care more efficiently and effectively with a multidisciplinary team. We are looking for individuals who are passionate about transforming our care system for people who need it most and are driven by doing well by doing what’s right.
The role of Director Edge Care team will focus on the following areas:
Serve as the care team leader responsible for refining, executing and overseeing the ideal care team for our members
Directly oversee 4 initial direct multidisciplinary reports (including all respective guild leads for care navigators, nurses, pharmacists and social work) over next 6 months, growing to oversight of a team of 30+ in multiple geographies within one year
Designing and executing pilots over the next 6 months to test and refine model and overseeing hiring of team to scale successful pilots over the next 1 year
Design and execute incentive system for care team to ensure that everyone is motived to ensure success for all of our members
Maintain budget oversight for care team and adjusting staffing ratios as necessary
Work with central team and marketing to open care centers in two markets
Work closely with Medical Director of on clinical aspects of care team
Work to identity what tech-enabled services will be necessary for team to provide necessary care and work with product team on development
Bachelor's Degree in any field
8 years of experience leading and growing multidisciplinary teams
This candidate must be an insightful risk-taker willing to thoughtfully disrupt the status quo and excited to work in a rapidly evolving start-up environment
Possess deep compassion for one’s customers, strong leadership and management skills and demonstrated ability to work in a multidisciplinary team
Passion for providing exemplary customer service to all our customers and partners
Passion for growing and developing ones reports and ensuring their growth and development
Comprehensive knowledge of all Microsoft Office applications, including Word, Excel and Access
Proficiency in analyzing and interpreting financial trends
Previous experience working as an Administrator overseeing multidisciplinary teams
Master's Degree in Business Administration or a related field
Administrative experience in a both direct care environments as well as call center environments
Working with seniors a plus
Clinical experience as a nurse, social worker, pharmacist, a plus
